Aviation Exams

Dr. Lawrence Foltz and Dr. Sherilyn Foltz-Cook are both Senior Aviation Medical Examiners (AME) for the FAA and offer exams for Class 1, 2, and 3 medical certificates.

Class 1

  • Valid for 6 months if the airman is age 40 or over on or before the date of examination.
  • Valid for 12 months if airman has not reached age 40 on or before the date of examination.

Class 2

  • Valid for 12 months

Class 3

  • Valid for 24 months if the airman is age 40 or over on or before the date of examination.
  • Valid for 60 months if the airman has not reached age 40 on or before the date of examination.

HOW TO APPLY FOR A MEDICAL CERTIFICATE

  1. Login to medexpress.faa.gov
  2. Complete the 8500-8 application and submit.
  3. Print the summary page, which will include the confirmation number.
  4. Call our office to schedule.

WHAT TO BRING TO THE APPOINTMENT

  1. Photo ID
  2. Confirmation number
  3. Any medical documents (if required). If unsure what medical documents are required, please call our office or look under the FAA AME GUIDE.
